Weight loss that is actually supervised.

Exam, labs and GLP-1 therapy prescribed by a physician assistant who sees you every month — not a form you fill in and a box that shows up.

A weight-loss program that starts with a physical exam.

Plenty of places will mail you a GLP-1. What they will not do is examine you, run your labs, check what else is going on, and sit with you every month while your body changes. That is the difference between a prescription and a program.

Twenty-One Medical is a physician assistant led medical clinic, not a med spa. Weight loss here is treated as medicine, because for most people that is exactly what it is.

How the program runs

STEP 01

Consult and exam

A real visit. Your history, your medications, what you have already tried, and what has gone wrong with it.

STEP 02

Labs

Bloodwork before anything is prescribed — including the thyroid, metabolic and hormone markers that quietly stall weight loss.

STEP 03

Your plan

If medication fits, it is prescribed and dosed for you. If something else is driving the weight, we treat that instead.

STEP 04

Monthly visits

After the initial consult you are seen monthly to evaluate your progress and make any adjustments needed.

What happens at each monthly visit

Every month you come in, we do a weigh-in, talk through your progress, and make any changes the numbers call for. You are then given your next month's worth of medication at that visit.

That is the reason this program runs in person rather than over video — the weigh-in and the hand-off of medication both have to happen in the office.

Medications we prescribe

When medication is the right tool, we use GLP-1 and dual-agonist therapy — semaglutide and tirzepatide. Which one, at what dose, and for how long is a clinical decision made at your visit, not something to shop for online.

There are also medications other than GLP-1s that may suit you better, or that your insurance may actually cover. Which applies to you is decided case by case.

Worth saying plainly: these medications are not right for everyone, and they are not a substitute for the rest of the work. They are most effective alongside the nutrition and activity changes we talk through at each visit.

Who this is for

  • People who have lost weight before and put it back on
  • People whose labs have never actually been looked at
  • Men and women whose hormones may be part of the picture — we treat that here too
  • Anyone who wants the same provider at every visit rather than whoever is on shift

What it costs

The program is a flat monthly fee that covers everything you need to reach your goal — the visits, the monitoring and the medication. The actual figure depends on which medication you end up on.

Common questions

Do I need labs before I start?

Yes. We will not prescribe a GLP-1 without bloodwork. It is how we catch thyroid and metabolic problems that would otherwise sabotage the whole program — and how we make sure the medication is safe for you specifically.

How often will I be seen?

Monthly. After the initial consult you come in once a month so your progress can be evaluated and the plan adjusted as needed. Follow-up and medication pick-up visits can be requested online.

Can this be done by telehealth?

Not generally. The weight-loss program runs in person because each visit includes a weigh-in, a discussion of your progress, and the hand-off of your next month's medication. Telehealth is available for other services, but not usually for this one.

Do you take insurance?

The program is generally self-pay. Most insurance plans do not cover weight-loss medication, and the plans that do tend to have very specific requirements. There are also medications besides GLP-1s that may be covered — that is assessed case by case. Call the office and we will tell you what applies to your plan.
